5140i Unlock codes

    Hi people,
    I've just changed network provider but I'd like to keep using my old 5140i on sites etc. I'd rather not try the online calculators myself as I'll no doubt screw something up.
    Phone details are below if anyone can help me it'd be much appreciated.
