I don't want to be a sceptic but you are asking to collect personal information without at least telling us what exactly you plan to do with it. There are threads here which you could peruse to obtain some of what you are looking for, but I think it would be prudent for you to clearly state your intentions about the "stuff" you need to fill out and what you plan to do with it. We're pretty helpful and friendly but we see a lot of suspicious behaviour so can't help but be a little weary at times. No harm meant.
I'm a student from the University of Essex, UK, and as part of my course, we are asked to complete a research project. This years project is titled 'Usage Of The Internet', and has been left open ended for us to decide what usage to report on. The only information i will be collectingf will be that stated above. No name, ID, address, IP address, telephone number etc will be recorded, only the age, hours spent and what the usage is (such as games, info, email etc). Once the report is completed, the information will only stay on the paper handed in. It is purely to gain marks for myself by showing that i am capable to gain information by means of a questionaire.
