Probably you are using 2x media, or 4x media which are fake. (say that they are 4x but they are just 2x) Try some good media (Ritek, TDK) from a trusted dialer and if this does not work try flashing the 1.07 hacked firmware from http://www.chrismccann.co.uk/pioneer_firmware_upgrades.htm This firmware will also let you rip with speeds up to 12x and burn at 2x to discs not supported by Pioneer. It is also region-free. (credit goes to Oriphus!)
