Need Unlock Code For 6010

I need your help obtaining an unlock code for my 6010. It came new in original packaging with the paperwork indicating it was intended for AT&T. Which is good because that's my provider.

Thank you soooo much! I have printed out these instructions and will take them to my local AT&T shop and let them do it. I tend to hit the wrong keys as often as not so I'd rather an "expert" did it.
