nokia 6126 restriction code

i have a nokia 6126. it is a t mobile phone but i have a cincinnati bell sim card. i need the restriction code for my phone to convert from t mobile to cincinnati bell. can anyone help?

 

I thought these were from Cincinnati Bell.

I don't think it can be done.
Save your time and find an unlocking service if Your provider will not help.
